  • Under the supervision of the Routt County Communities That Care (CTC) Facilitator, the Youth Development Coordinator (YDC) is responsible for the coordination of youth driven community-based substance abuse prevention services within Routt County, specifically in South Routt and Hayden, according to the Communities That Care (CTC) contracted Scope of Work. The YDC collaborates with the Routt County CTC Coalition Board, staff from youth-serving organizations, and school districts to fulfill the CTC Action Plan and acts directly to support the CTC Facilitator. Education: Minimum of bachelor’s degree or 2 years’ experience in work-related field preferred Experience: Experience working directly with young people ages 9-25 preferred. Experience with needs assessment, logic model construction, selection and implementation of research-based prevention interventions and staging intervention components.   • As needed position in Moffat and Routt County. Provides clinical and educational services to clients of the Health Center. Acts as a resource to professional and support staff. Education: Successful completion of a Medical School program including residency or completion of an accredited Physician Assistant program or a Bachelor Degree in nursing by a school accredited by the National League of Nursing and a family planning or adult nurse practitioner certificate.   • Full time, benefitted position for Northwest Colorado Health. This position will participate in the implementation of the dental services in the Community Health Center and will also practice dental hygiene clinically. This position will integrate oral health care into the comprehensive delivery system of Team Based Care within the CHC and the community. Education: Graduation from an accredited dental hygiene school is required. Bachelor’s degree in Dental Hygiene is preferred. Experience: Previous experience with serving ethnically and culturally diverse patient populations across the lifespan preferred.   • This is a full time position. A Dental Assistant is a vital member of the dental health team. As a dental assistant, you will enjoy a broad range of exciting and challenging duties. Dental Assistants prepare the patient for the treatment, assist the dentist in performing their treatment to the patient in restorative dentistry or oral surgery, prepare materials and equipment for treatment and have them ready for the dentist’s use, take x-rays and assist the dentist in laboratory work. May perform other related work to include, but not limited to, making appointments, answer the phone, ordering supplies and doing the dental billing.   • Per diem position based at The Haven Assisted Living facility. This position is responsible for the nutritional needs of the residents, including three (3) meals per day plus snacks. In addition, the facility provides food services for special events and meetings. All food services will be prepared in accordance with State Health Department guidelines as to proper clothing and hair restraints, proper temperatures of food at time of serving, proper cleanliness and general good kitchen/pantry conditions.
