  • Full time position based in Routt County. In close cooperation with the Provider team of the Community Health Center provides a variety of direct services to clients of the Health Center and assists them to obtain needed specialty care and ancillary health care services. Education: Bachelor’s degree preferred. Experience: One to two years clinical experience, ideally some Case Management /care coordination work. Bilingual English and Spanish preferred.   • PRN or “as needed” position based at The Haven. The Caregiver is responsible for providing a physically safe and sanitary environment, personal services, protective oversight, and social care sufficient to meet the needs of residents. Education: High School diploma or equivalent preferred but not required. Experience: Previous caregiving experience either in Assisted Living or other health care setting preferred.   • PRN or “as needed” position based at The Haven Assisted Living facility. This position is responsible for the nutritional needs of the residents, including three (3) meals per day plus snacks. In addition, the facility provides food services for special events and meetings. All food services will be prepared in accordance with State Health Department guidelines as to proper clothing and hair restraints, proper temperatures of food at time of serving, proper cleanliness and general good kitchen/pantry conditions. Education: High School diploma or equivalent required. Experience: Previous meal preparation, commercial cooking, and planning experience is preferred.   • This is a PRN position that serves both Routt and Moffat Counties. The Aging Well Program provides comprehensive community-based healthcare and social services that promote health and wellness for older adults in rural areas. The Foot Care Program provides foot care and foot health education, to screen and make appropriate referrals for foot problems such as foot ulcers and loss of protective sensation; to enable case finding among the senior population and those with chronic disease for other problems, and to make appropriate referrals when additional care is needed. Education: A Baccalaureate Degree in nursing by a school accredited by the National League of Nursing is preferred. Experience: Proven physical assessment skills are preferred.   • This is a PRN position in both Routt and Moffat counties. This is an advanced performance level of social work practice, providing services to patients and families of patients in their homes relative to the physical and mental health of the patients, family involvement and community organization for assessment, care planning, treatment and coordination on behalf of patient's ongoing medical care and/or rehabilitation. Education: Must have a Medical Social Worker degree from a school of social work accredited by the Council on Social Work Education. A Licensed Clinical Social Worker (LCSW) is preferred. Experience: One year of social work experience in a health care setting is preferred. The licensed salary range for this position is $27.22-$35.33, non-licensed $18.63-$23.00 and is eligible for our 403(b) Retirement Plan. View a complete job description.
