  • This is a PRN or as needed position (16 hours/week) located in Moffat County. Responsible for the coordination of spiritual support and bereavement services to patients and families in keeping with the patient and family's desire and belief system, and consistent with the policies and procedures of the hospice program. Education: Graduate from an accredited seminary or school of theology or appropriate certification in hospital or pastoral ministry. Experience: Three to five years of experience as a spiritual care coordinator or chaplain, preferably with an emphasis on terminally ill.   • This is a full time, exempt position covering Routt and Moffat Counties. The Youth Resiliency Program Manager is responsible for planning, directing and monitoring the coordination of the day to day operations of the Youth Resiliency Program, including direct services, staff supervision, policy management, public relations and budgetary management. This position also provides direct services to clients of the YR program which include providing and ensuring high quality trauma informed service delivery in all aspects of youth care through direct supervision, management, and program development. The YR Manager will build intentional relationships with youth, engage them in a variety of programming and work through their stages of change in order to drive toward long term positive outcomes. Education: Master’s Degree in Social Work, or related field preferred. Experience: 3-5 years of project and program management experience. Experience with group facilitation and teaching youth. Experience with trauma and youth. Master’s level clinical experience preferred.   • Full time-30 hours a week position in Moffat and Routt Counties. A Dental Assistant is a vital member of the dental health team. As a dental assistant, you will enjoy a broad range of exciting and challenging duties. Dental Assistants prepare the patient for the treatment, assist the dentist in performing their treatment to the patient in restorative dentistry or oral surgery, prepare materials and equipment for treatment and have them ready for the dentist’s use, take x-rays and assist the dentist in laboratory work. May perform other related work to include, but not limited to, making appointments, answer the phone, ordering supplies and doing the dental billing. The Aging Well Program provides comprehensive community-based healthcare, fitness and education services to older adults in rural areas. Instructors are assigned to teach evidence-based programs and/or fitness classes promoting the health and wellbeing of older adults. Education and/or Experience: Minimum requirements include knowledge and training, including appropriate professional certifications in exercise and fitness and BLS. A certification through the Arthritis Exercise Association (AEA) or other high tier evidence-based health promotion /disease prevention program as defined by the Administration on Aging within 3 months of hire. In addition, the facility provides food services for special events and meetings. The Aging Well Program provides comprehensive community-based healthcare and social services that promote health and wellness for older adults in rural areas. The Foot Care Program provides foot care and foot health education, to screen and make appropriate referrals for foot problems such as foot ulcers and loss of protective sensation; to enable case finding among the senior population and those with chronic disease for other problems, and to make appropriate referrals when additional care is needed. Education: A Baccalaureate Degree in nursing by a school accredited by the National League of Nursing is preferred. Experience: Proven physical assessment skills are preferred.   • This is a PRN position in both Routt and Moffat counties. This is an advanced performance level of social work practice, providing services to patients and families of patients in their homes relative to the physical and mental health of the patients, family involvement and community organization for assessment, care planning, treatment and coordination on behalf of patient's ongoing medical care and/or rehabilitation. Education: Must have a Medical Social Worker degree from a school of social work accredited by the Council on Social Work Education. A Licensed Clinical Social Worker (LCSW) is preferred. Experience: One year of social work experience in a health care setting is preferred.
