COVID-19 Vaccine
Anyone 6 months and older is eligible to receive the COVID-19 vaccine in Colorado. We are currently providing:
- Moderna COVID-19 vaccine - patients must be 12 or over to receive this vaccine - primary series only
- Pfizer COVID-19 vaccine - patients must be 6 months old or over to receive this vaccine; monovalent boosters available for those ages 5-11
- Pfizer Bivalent COVID-19 booster - patients must be 12 years old or over to receive this booster dose and must show proof of primary 2 dose vaccine series
CDC recommendation states that the Bivalent Booster can be administered as the booster dose 8 weeks after the primary series and 8 weeks after a previous booster dose for those ages 12+. If you had COVID-19 it is recommended to wait 3 months from when symptoms started or a positive test to receive the next scheduled booster. The monovalent booster still remains the recommended booster for ages 5-11, and no booster for those 6 months – 4 years.

What to expect at your vaccine appointment

The Moderna COVID-19 vaccine requires 2 doses, 28 days apart. The Pfizer COVID-19 vaccine requires 2 doses, 21 days apart. When you come in for your first vaccine appointment, you will be required to schedule your second vaccine appointment.

COVID-19 vaccine appointments are 20 minutes long and patients are required to wait inside the clinic during the observation period after receiving the vaccine.

Patients must sign a consent form for both 1st and 2nd doses of the vaccine at the time of their appointment.

Every effort should be made to make your scheduled vaccine appointment. Once a vaccine vial is opened, it must be used the same day. If you need to cancel your appointment, please call our clinic or cancel via your myClinicOnline account.

Patients should wear clothing that allows easy access to their upper arms, a few inches below the shoulder. Masks are required.

The COVID vaccine will be provided regardless of ability to pay. Medicare, Medicaid, CHP+ and private insurance will cover the cost of COVID-19 vaccine; please bring your insurance card to your appointment. There will be no charge for the uninsured.

The CDC requests that patients report any vaccine side effects through v-safe.
COVID-19 Testing
We offer COVID-19 testing at our Community Health Centers in Craig and Steamboat Springs. Standard PCR tests and rapid tests are available depending on your situation. Costs vary; discounts available for those without insurance. Please bring your insurance card to your appointment.
Please note: Due to limited staff time and resources and an increasing COVID-19 positivity rate, we are not able to provide COVID-19 testing for travel purposes, whether it is a rapid test or PCR test. If you are in need of a COVID-19 test for international and airplane travel requirements, please look to Moffat County and Routt County Public Health for additional testing resources in our area. Take-home rapid tests are available for free at our Community Health Centers.
