School-Based Health
Northwest Colorado Health is collaborating with Hayden, Steamboat Springs and South Routt School Districts, and the Boys and Girls Clubs of Northwest Colorado, to bring School-Based Health services to our community. This program is a part of Northwest Colorado Health’s Community Health Center Program and provides dental services in our mobile unit, as well as behavioral health services by referral from individual schools. Medicaid and Private Insurance accepted. If you do not have insurance, the amount you pay for services will be based on your family size and income. Summer appointments are available from 8am-4pm on the following days and locations in our mobile dental unit. To request an appointment please call 970-846-4426. Please note that appointments at the Boys & Girls Club in Craig are open to all community members, children do not need to be attending the Boys & Girls Club to make an appointment.
