School-Based Health
Northwest Colorado Health is collaborating with Hayden, Steamboat Springs and South Routt School Districts, and the Boys and Girls Clubs of Northwest Colorado, to bring School-Based Health services to our community. This program will be part of Northwest Colorado Health’s Community Health Center Program and will initially provide dental services. Medicaid and Private Insurance accepted. If you do not have insurance, the amount you pay for services will be based on your family size and income. To request an appointment please call 970-846-4426.

Northwest Colorado Health está colaborando con los Distritos Escolares de Hayden, South Routt y Steamboat Springs, y Boys and Girls Club en Craig, para traer servicios de salud basados en la escuela a nuestra comunidad. Este programa formará parte del Programa de Centros de Salud de la Comunidad de Northwest Colorado Health e inicialmente proporcionará servicios dentales. Se aceptan Medicaid y seguros privados. Si no tiene seguro, la cantidad que pague por los servicios se basará en el tamaño de su familia y sus ingresos.
