Home Health

We provide skilled, compassionate and personalized care in the comfort and security of familiar surroundings. Our Home Health team includes registered nurses, physical and occupational therapists and medical social workers. We address your home care needs and serve as advocates, answering questions and contacting your medical provider with concerns and connecting you to community resources.  

We provide Home Health and Hospice services in Routt and Moffat counties. Home Health is available through a physician's referral. Call us for more information if you are considering Home Health for yourself or a loved one. We will work with your medical provider. We accept Medicare, Medicaid and private pay. Personal Care Providers

Personal Care Providers provide in-home assistance with non-medical needs such as personal care, bathing, shaving, light housekeeping, grocery shopping, meal prep, running errands and more. This service can be a good respite option for family members who are caring for disabled, sick or elderly loved ones. Personal Care Provider service is $45 to $65 per hour. Private Pay Services

Our Private Pay Services are for individuals who want help with personal care in their home, or have graduated from Home Health and would like to continue services. This is a private pay service in Routt and Moffat counties that does not require a doctor's referral.

  • Nursing services such as wound care, ostomy care, IV therapy, catheter care, medication management and monitoring vitals/general health
  • Physical Therapy services such as home safety evaluation, strengthening, transfer training, walking program, and fall prevention
  • Medical Social Worker services include coordinating resources to address patients’ physiological, safety and social needs
  • Support for daily living and non-medical needs such as bathing and shaving, light housekeeping, meal preparation, grocery shopping and running errands and transportation to appointments
