The Client Assistance Program allows you and others in your household to receive services at Northwest Colorado Health on a sliding fee scale. During the new patient process, you will complete an application and you may be assigned a Fee Code on the Sliding Fee Scale. The amount you pay for services will be based on your family size and income. The Client Assistance Program covers services provided by the clinic. If you need services that are not covered, you will be informed and will be made aware of the cost before services take place.
