Newly enrolled children and pregnant women in Health First Colorado and Child Health Plan Plus (CHP+) have a family health coordinator available to assist them in finding resources to connect to care. A family health coordinator directs newly enrolled members on Health First Colorado and CHP+ to primary and specialty medical providers, community-based organizations and agencies services. Please call 970-871-7693 to be connected to a family health coordinator.
