Cardiovascular and Diabetes Free Screenings

Our free screenings help clients gauge their heart health and risk for cardiovascular disease and diabetes. Based on results, our Community Health Educators provide lifestyle recommendations and resources to help prevent heart attack, stroke and diabetes.

After the initial screening, you can continue to meet with our Community Health Educators, who will work with you to create a personalized plan tailored to meet your own goals and fit into your schedule. Choose what works for you from options such as incorporating movement into your everyday life and learning about the foods you eat. We help you integrate one small habit at a time that is sustainable, knowing that small changes over a lifetime add up to improve health. Follow up sessions may occur in person, over the phone or virtually.

  • Dietary and Nutritional Assistance
  • Exercise Plans
  • Personalized Health Improvement Plan
  • Supplemental Education and Resources

Individuals who receive screenings will receive reminders for free follow up screenings based on their risk. To make an appointment or to inquire about offering free screenings to employees at your business, call 970-871-7692 in Craig or 970-871-7694 in Steamboat Springs.

Drop-In Screenings in Craig

Come in for a free cardiovascular screening - no appointment necessary! Our team will host monthly drop-in hours for community members to receive their screening. You do not need to be a current patient to use these services, let the front desk know you are there for the Drop In Screenings. 

Drop-in hours will take place at our Community Health Center in Craig (745 Russell St.) on the following days: 

  • 1st Tuesday of every month (starting August 4, 2026) from 9am-2pm
  • Last Wednesday of every month from 9am-2pm
