Our free screenings help clients gauge their heart health and risk for cardiovascular disease and diabetes. Based on results, our Community Health Educators provide lifestyle recommendations and resources to help prevent heart attack, stroke and diabetes.
After the initial screening, you can continue to meet with our Community Health Educators, who will work with you to create a personalized plan tailored to meet your own goals and fit into your schedule. Choose what works for you from options such as incorporating movement into your everyday life and learning about the foods you eat. We help you integrate one small habit at a time that is sustainable, knowing that small changes over a lifetime add up to improve health. Follow up sessions may occur in person, over the phone or virtually.

Free Community Education Classes
In addition to our regular free screenings and education, we also occasionally offer community education classes. Led by our Registered Dietitian, the Heart Healthy class will cover topics such as: knowing the signs of heart attack and stroke, diabetes and cholesterol nutrition education, shopping tips and more!
