Senior wellness checks are offered on a donation basis. Visits include weight and blood pressure check, medication review and consultation with a nurse. Drop ins are welcome. For more information, call 970-871-7676.

We’re so proud to have started and grown the Aging Well Program in Northwest Colorado. Starting July 1, 2022 our partners at Routt County Council on Aging will facilitate Aging Well  in Steamboat Springs, Oak Creek and Hayden. Northwest Colorado Health will continue to operate Aging Well Programs in Moffat County. Due to a lack of staff and funding, Northwest Colorado Health will no longer offer Footcare Services for Older Adults in Routt and Moffat Counties effective March 1, 2023.
