Northwest Colorado Health is seeking a Chaplain with the passion and skills to coordinate spiritual support and bereavement services to patients and families in keeping with the patient and family's desire and belief system and be consistent with the policies and procedures of the hospice program. We have been providing Home Health and Hospice services for more than thirty years. Today, you will join a healthcare organization and staff in two counties providing skilled, compassionate and personalized care in the home. We have high expectations for you as a Chaplain. We need you to:

• Offer and/or provide spiritual support to meet patient and family needs in accordance with the patient’s and family’s acceptance of this service, and in a manner consistent with patient and family beliefs and desires.
• Offer and/or provide bereavement support to meet patient and family needs in accordance with the patient’s and family’s acceptance of this service, and in a manner consistent with patient and family beliefs and desires.
• Participate in patient conferences as a member of the interdisciplinary team to explore and assess the potential spiritual and bereavement needs of patients and families and reporting on services and responses with measurable goals and outcomes.

We offer a salary that meets market values for the Colorado area and is based on experience. The salary range for this PRN or “as needed” based in Moffat County is $26.19-$35.75 and is eligible for our 403(b) Retirement Plan. View a full job description. EOE. Open until filled.
