Northwest Colorado Health is seeking a Home Health Coordinator with the passion and skills to deliver high quality healthcare. We have been providing Home Health and Hospice services for more than thirty years. We have high expectations for you as a Home Health Coordinator. We need you to:

• Be responsible for the quality and content of the medical record, from admission to discharge.
• Organize, maintain, scan and file medical records documents using developed and established systems as set by the agency.
• Perform general office duties and all clerical duties related to the processing of requests related to the electronic health record, physician/ provider orders and skilled staff (i.e. answering telephone, typing, submitting fax requests and all miscellaneous clerical as delegated by the (Clinical Supervisor).
• Create and maintain client medical record in electronic health record.
• Track and follow-up on delinquent medical records until complete.
• Requisition medical records forms; collates new permanent and patient information folders.

We offer a salary that meets market values for the Colorado area and is based on experience. The salary range for this full time, 40 hour a week position located in Moffat and Routt Counties is $20.16-$29.63 and is eligible for Medical, Dental, Vision, Life Insurance, Long-Term Disability, Short-Term Disability, Paid Time Off and our 403(b) Retirement Plan.
