Birth and Death Certificates
Vital records in the state of Colorado are confidential. Applicants requesting certified copies must provide acceptable identification as well as proof of relationship or direct and tangible interest. Applications and additional information are available below or by contacting us at:

Our Moffat County Vital Statistics Team was recognized as one of only ten recipients of the 2020 Five Star Performance Award in the state! This award sets a high performance standard for all vital records offices within Colorado and it is a great accomplishment to have fulfilled the criteria.
Northwest Colorado Health (Routt County)
Starting August 1, 2019, Routt County will begin to issue both birth and death certificates to residents at the Clerks & Recorder office. To obtain Vital Records you will need to visit the Routt County office at 522 Lincoln Ave. in Steamboat Springs from 8:00 a.m. – 4:30 p.m. Monday – Friday. Birth Certificates
Birth Certificates for individuals born anywhere in the state of Colorado may be obtained from the Northwest Colorado Health offices in Moffat County by completing the required application and providing acceptable identification as well as proof of relationship or direct and tangible interest. Birth Certificates are $20 (search fee & covers cost of 1st certified certificate if produced) and $13 for each additional copy. Applications will be accepted Monday through Friday, but all the applications that come in on Friday will not be ready until Monday. Birth Certificates are not available on a walk-in basis.
Death Certificates
Northwest Colorado Health maintains the records of Moffat County deaths. Moffat County Death Certificates prior to 2015 may only be obtained in these respective counties. Colorado death certificates 2015 to present may be obtained from any vital records office in the state, regardless of the county in which the death took place. Death Certificates may be obtained from the Northwest Colorado Health offices by completing the required application and providing acceptable identification as well as proof of relationship or direct and tangible interest. Death Certificates are $20.00 for the first copy and $13.00 for each additional copy. Applications will be accepted Monday through Friday, but all the applications that come in on Friday will not be ready until Monday. Death Certificates are not available on a walk-in basis.
