Do you serve patients without insurance?

Yes. We welcome all patients regardless of insurance status or ability to pay. Many services are offered on a sliding fee scale based on income and family size.

Where are your Community Health Centers located?

Our Community Health Centers are located in Craig at 745 Russell Street and in Steamboat Springs at 940 Central Park Drive, Suite 101.

The 35th annual Daffodils fundraiser in March raises important funds for our Hospice program in Routt and Moffat counties.
