Behavioral Health Provider (Steamboat Springs & Craig)
Northwest Colorado Health is seeking a full-time Behavioral Health Provider with the passion and skills to deliver high quality integrated healthcare. We have been providing primary healthcare to residents since 2008. Today, you will join a healthcare organization and a staff of 11 providers at four locations, delivering primary medical, dental and behavioral health care to more than 5,500 patients. We have high expectations for you as a Behavioral Health Provider. We need you to:
- Provide integrated behavioral health as a key member of the primary care team.
- Abide by guidelines and expectations specific to the clinic in which they provide patient care.
- Provide brief evidence-based treatment for psychiatric disorders, chronic medical conditions, and emotional/behavioral dysregulation that does not meet criteria for a psychiatric condition.
- As needed, the BHP provides case management, consultation, care coordination, and crisis risk assessment, and intervention.
- Provide evidence-based screening, assessment, treatment planning, and risk assessment of behavioral health conditions
- Consult with primary care or dental providers to improve patient functioning and health status.
- Provide patient care within scope of services provided within the CHC.
- Remain current with standards of mental and behavioral health practice.
- Work with interdisciplinary team to provide team-based care as part of the comprehensive primary care services provided within the CHC. Conducts independent behavioral health visits and integrated care visits during medical and dental visits to increase patient access to care.
- Participate in the Performance Improvement Process
- Participate in program development when needed as directed by Behavioral Health Director.
- Participate in the review on outcomes of individual client interventions as well as general program effectiveness.
- Participate in performance improvement and quality improvement efforts.
- Demonstrate dedication to meeting encounter targets on an individual and team level. Completes documentation and requirements supporting financial health of organization.

We offer a salary that meets market values for the Colorado area and is based on experience. The salary range for this full-time position Routt and Moffat Counties is $65,041.60 – $104,062.40, depending on licensure and is eligible for Medical, Dental, Vision, Life Insurance, Long-Term Disability, Short-Term Disability, Paid Time Off and our 403(b) Retirement Plan.
