Northwest Colorado Health is seeking two School Based Behavioral Health Providers to serve Routt County with the passion and skills to deliver high quality Behavioral Health Services to our schools. One full time and one part time position. We have been providing primary healthcare to residents since 2008. Today, you will join a healthcare organization and a staff of 11 providers at four locations, delivering primary medical, dental and behavioral health care to more than 5,500 patients. We have high expectations for you as a Behavioral Health Provider. We need you to:

• Provides brief evidence-based treatment for psychiatric disorders and emotional/behavioral dysregulation that does not meet criteria for a psychiatric condition.
• Provides evidence-based screening, assessment, treatment planning, and risk assessment of behavioral health conditions.
• As needed, the BHP provides case management, consultation, case coordination to facilitate referrals to specialty care, and crisis intervention.
• Remains current with standards of mental and behavioral health practice.
• Coordinates care with Community Health Center and School systems.  When coordinating with individuals outside of the CHC, BHP is sure to align with all privacy laws and legal/ethical guidelines.
• Participates in the Performance Improvement Process
  • Participation in program development as is needed by behavioral health team
  • Participate in the review on outcomes of individual client interventions as well as general program effectiveness.
  • Participates in performance improvement and quality improvement efforts.
• Demonstrates dedication to meeting encounter targets on an individual and team level. Completes documentation and requirements supporting financial health of organization.

We offer a salary that meets market values for the Colorado area and is based on experience. The salary range for the full-time position covering Routt County is $62,379.20-$98,092.80, depending on licensure and is eligible for Medical, Dental, Vision, Life Insurance, Long-Term Disability, Short-Term Disability, Paid Time Off and our 403(b) Retirement Plan. The salary range for the part-time position covering Routt County is $31,189.60-$49,046.40, depending on licensure and is eligible for Paid Time Off and our 403(b) Retirement Plan.
