Jo Anne Grace, Ph.D. is the Hospice Chaplain - Spiritual Care and Bereavement Coordinator. Spiritual Care is an integral component of Hospice. It is based on compassionate listening and reflective dialogue and is focused on finding meaning and kindling hope for Hospice patients and families. Bereavement is also provided to Hospice families for one year following a patient's death. In addition to the Bereavement program available to all Hospice patients and families, Dr. Grace coordinates Community Grief Support which is open to anyone in the community; including: Community Grief Support Groups, the Grief Resource Center, Counseling and Group Education. In addition to her role at Northwest Colorado Health Dr. Grace is a pastoral theologian, a spiritual director, and a health educator. Her private practice focuses on Pneumasomatic Care - a ministry of reconciliation and healing.
