With well over two decades of nursing experience, Jennifer is a Registered Nurse specializing in home health and hospice, long-term care, and surgical recovery. Currently serving as Intake Coordinator at Northwest Colorado Health, she evaluates referrals, coordinates in-home care services, and ensures compliance with CMS regulations. Her background includes leadership as a Charge Nurse in long-term care, agency experience across surgical and rehabilitation units, and extensive work in hospital-based surgical and medical/surgical units. She has strong experience and knowledge in wound care as well. Jennifer is deeply committed to providing compassionate support to patients and their loved ones throughout their hospice journey. She brings a consistent focus on patient-centered care, clinical excellence, and inter-disciplinary collaboration.
