Elise Sullivan, MD - Physician

Elise Sullivan, MD - Physician

Elise Sullivan, MD, is a board-certified family medicine physician with more than 14 years of experience providing comprehensive primary care to patients with a wide range of healthcare needs. She is committed to delivering high-quality, patient-centered care and takes time to understand each patient’s concerns, circumstances and health goals.

Education and Medical Training

Dr. Sullivan graduated from the University of Colorado School of Medicine in 2007, where she focused on rural healthcare. She completed her residency at Santa Rosa Family Medicine in 2010 with an emphasis on community health and caring for underserved populations.

She is board-certified in family medicine and enjoys helping educate the next generation of healthcare professionals by teaching medical students.

Clinical Interests

Dr. Sullivan’s clinical interests include:

  • Pediatric and adolescent healthcare
  • Adult and geriatric primary care
  • Women’s health
  • Long-acting reversible contraception, including IUDs and Nexplanon
  • Cervical and breast cancer screening
  • HIV and hepatitis C treatment
  • Transgender healthcare
  • Substance use disorder treatment
  • Acupuncture

Approach to Patient Care

Dr. Sullivan believes in a holistic and collaborative approach to medicine. She listens carefully to her patients and works with them to develop individualized treatment plans based on their needs, preferences and health goals.

As part of Northwest Colorado Health’s integrated care team, she helps connect patients with appropriate primary care, children’s healthcare, women’s healthcare and behavioral health services.

Community Involvement

A native of Pine, Colorado, Dr. Sullivan has been actively involved in the Moffat County community. She has served on the Moffat County Board of Education and volunteered through her church. Outside of work, she enjoys walking her dog, camping, snowshoeing and skiing.
