We provide integrated primary care, dental care, behavioral health services, home health and hospice, assisted living options and a wide range of prevention and wellness programs for people of all ages.

Do you serve patients without insurance?

Yes. We welcome all patients regardless of insurance status or ability to pay. Many services are offered on a sliding fee scale based on income and family size.

Where are your Community Health Centers located?

Our Community Health Centers are located in Craig at 745 Russell Street and in Steamboat Springs at 940 Central Park Drive, Suite 101.

How do I make an appointment?

To schedule an appointment, call 970-824-8233 in Craig or 970-879-1632 in Steamboat Springs, or use our online patient resources to get started.
